MAY 7, 2004
I HAVE BEEN FEELING TIRED AND JUST NOT MYSELF FOR ABOUT A YEAR NOW. I HAVE GAINED
A LOT OF WEIGHT, MY HAIR IS DRY AND BRITTLE, MY NAILS BREAK OFF FOR NO REASON AT ALL. I AM SLEEPY ALL THE TIME BUT WHEN I
TRY TO SLEEP I CAN'T.
I FINALLY FOUND A DOCTOR THAT WILL ACCEPT MY MEDICAID WITH SSI.
I GO SEE HIM ON THE 19th OF THIS MONTH.
I PRAY HE WILL HAVE SOME ANSWERS FOR ME AND I CAN START FEELING BETTER.

MAY 19, 2004
WENT TO THE DOCTOR TODAY.
HE THINKS IT MIGHT BE HYPOTHYROIDISM.
HE SENT ME ACROSS THE OFFICE TOWER TO AN IN HOUSE LAB TO GET BLOOD WORK DONE.
SHOULD FIND OUT THE RESULTS TOMORROW.

MAY 20, 2004
DOCTOR CALLED. IT IS FOR SURE HYPOTHYROIDISM.
HE SAID IT'S EASILY TREATABLE.
HE PUT ME ON LEVOXYL 0.75 mg TO START OUT WITH.
HE WANTS TO DO BLOOD WORK EVERY MONTH FOR THE FIRST 3 OR 4 MONTHS TO MAKE SURE 0.75
mg IS ENOUGH.
I GO BACK FOR A RECHECK AT THE END OF JUNE.

JUNE 17, 2004
SLIPPED AND FELL AND LANDED ON MY LEFT KNEE. IT'S VERY VERY BRUISED AND LOOKS
AWFUL! FEELS EVEN WORSE!
CALLED THE DOCTOR ABOUT IT AND WAS TOLD TO KEEP MY APT. ON THE 21st OF JUNE.

JUNE 21, 2004
WENT TO THE DOCTOR FOR MY HYPOTHYROIDISM RECHECK AND TO HAVE MY INJURED KNEE
LOOKED AT.
DOC IS HAPPY WITH MY THYROID MEDICATION
SENT ME TO THE HOSPITAL TO GET A KNEE X-RAY. X-RAY SHOWED THAT NOTHING IS BROKEN
AND MY PLATES AND SCREWS ARE IN PLACE.
IT'S JUST A REALLY REALLY BAD BRUISE. MY ENTIRE KNEE IS BLACK AND PURPLE...YUCK!

JANUARY 2005
DOCTOR ENDED UP ADDING ANOTHER 25mg TO MY THYROID MEDICATION. IT JUST WASN'T LEVELING
OUT ENOUGH.
ALSO ADDED A HIGH DOSE OF IRON PILLS AFTER BLOOD WORK RESULTS AND ALSO PUT ME
ON QUINNE FOR LEG TWITCHES.

MARCH 5,2005
Went to the doctor for a recheck on Thyroid. More bloodwork showed that the
iron is helping but not enough for me to stop taking it. My Thyroid is also still off so we upped the dose again. I'm now
taking 125mg as well as the iron and all my other normal meds..

March 9, 2006
Fell in kitchen floor. Spent the entire day in the ER getting x-rays. Doctor
is sure I broke my foot...but sense it's my flopped/dropped foot (dropped feet have different bone formations) with all the
nerve damage she put me in a cast. Also is forwarding me to a Ortho Specialist...someone that will know more about my previous
conditions and how to care for this foot....

March 15, 2006
Went to Ortho Surgeon. (Dr. F from Baylor in Dallas...the doctor that saved
my legs and life in 2000) After more x-rays I was told it was a very bad break. Sense I already have problems
with this foot and where the break is, I was put into a walking boot for 12 weeks. The normal is 6 to 8 weeks.
Then recheck with x-rays.

June 9, 2006
Recheck broken foot at Orth. Surgeons office. Everything looks good,
but it is growing H.O. like my other breaks did. Will cause some pain due to the H.O. and he's sure sense the foot already
has arthritis this will only make it worse.
No more rechecks needed unless I have problems.
